Swift famously gave $500,000 (341,000) to victims of the 2010 Nashville floods and also took part in George Clooney's 'Help For Haiti' telethon.
Northwest Tennessee Disaster Services director Rob Adcock praised Swift's activism, saying: "Taylor Swift has inspired the small community of Northwest Tennessee with her uplifting words and music of encouragement and has become a positive role model for the young and old alike here.
"It is with great honour and appreciation that we present this one of a kind, jewelled award to such an outstanding individual on behalf of our organisation."
Swift also recently donated $4m (2.5m) to the Country Music Hall of Fame and Museum to help fund music education.
The Hall of Fame in turn opened an exhibit dedicated to Swift's 'Speak Now World Tour', which is open to visitors through November 4.
